The Health Pass Program was developed by our administrators realizing the need for medical insurance and the number of families that cannot afford it in our community and local surrounding communities. This is not an insurance plan as it does not cover anything outside of our facility. It does however provide some basic medical coverage until the patient becomes eligible for medical insurance or an Illinois Medical Card.

• There is a monthly premium of $50.00 that gets deducted from a credit card or debit card once a month.
• A co-pay of $35.00 for each visit.
• This plan covers 95% of the labs and testing that we do in the office so there will be no additional fees.
• For all lab procedures not covered the patient will receive a 40% discount.
• Patient received 2 free prescriptions with each visit if it is one that we carry in our clinic.

At patients first visit there is a total charge of $125.00 which includes:

• first months premium of $50.00
• first co-pay of $35.00
• one time registration fee of $35.00

The first visit may be paid by cash, credit, debit card or by check.  The patient will sign a contract for a term of 6 or 12 months. The policy may be cancelled at any time, but a cancellation fee of $200.00 will be added.  The cancellation charge will be waived if the patient becomes eligible for insurance or for the Illinois Medical Card and the patient remains with us as their Primary Care Physician.

For all testing needs that wwe cannot provide such as CT, MRI, Mammogram, etc. we  collaborate with (MCAI) Midwest Center for Advanced Imaging. They provide a substancial discount for our Health Pass Patients as well as our cash patients.
